About the position

As a Home Health Physical Therapist at CenterWell, you will play a crucial role in providing skilled physical therapy treatment and training to patients with various injuries, illnesses, and functional disabilities. Your primary goal will be to help patients achieve the highest level of physical function through personalized rehabilitation programs. You will collaborate with physicians and clinical teams to develop and adjust treatment plans, supervise support staff, and ensure thorough documentation of patient care.

Responsibilities

• Plan and administer prescribed skilled physical therapy treatment and training for patients.
• Test/screen the patient's physical strengths to assist in evaluating their level of function.
• Develop and implement a conditioning/rehabilitation program consistent with the physician's Plan of Treatment.
• Confer with physician and clinical team members to obtain additional patient information and assist in developing treatment programs.
• Provide written instructions/care plans to Physical Therapy Assistants and Home Health Aide staff.
• Supervise and evaluate the performance of Physical Therapy Assistants and Home Health Aides.
• Accurately document patients' care observations, interventions, and evaluations.
• Report patient's progress to the physician, Clinical Manager, staff, patient, and family.

Requirements

• Degree from an accredited Physical Therapy Program (approved by the APTA).
• Minimum of one year physical therapy experience preferred.
• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Strong organizational and communication skills.
