About the position

UPMC Home Health is seeking a full-time homecare Physical Therapist to provide in-home care to patients in Millvale and surrounding areas. The role involves establishing treatment plans, collaborating with healthcare professionals, and ensuring optimal patient care while maintaining accurate documentation. Responsibilities

• Perform duties in alignment with UPMC Home Healthcare's service management philosophy.
• Participate in on-call coverage rotation for the agency.
• Assign and supervise the Physical Therapy Assistant according to state guidelines.
• Consult with families, interdisciplinary team members, and physicians to ensure optimal patient care.
• Sync data and maintain accurate documentation including visit assessments and regulatory standards.
• Establish plans of care in collaboration with physicians and patients based on orders and needs.
• Evaluate, plan, and treat patients using effective physical therapy programs.

Requirements

• Graduate from an accredited physical therapy curriculum or equivalent qualifications.
• Hold a valid Physical Therapist License.
• CPR certification based on AHA standards within 30 days of hire.
• Possess a valid Driver's License and Automotive Insurance.
• Clearances: Act 33, Act 34, OAPSA.
