About the position

The Physical Therapist will provide outpatient rehabilitation services primarily in an in-home setting for older adults. This role focuses on delivering patient-centered care that enhances recovery and promotes independence through individualized therapy programs. The therapist will assess patient needs, develop treatment plans, and collaborate with other healthcare professionals to ensure comprehensive care.

Responsibilities

  • Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel as required.
  • Adhere to department and patient treatment schedules, ensuring timely clinical and billing documentation.
  • Attend and contribute to patient care and staffing conferences.
  • Make referrals to other disciplines and agencies to address comprehensive patient needs.
  • Consult with rehabilitation team members to select appropriate therapeutic regimens.
  • Adapt programs and plans of care according to individual patient needs.
  • Promote a safe environment and eliminate fire and safety hazards.
  • Order necessary supplies and equipment.
  • Maintain and report the need for repair of equipment.
  • Ensure high-quality service for patients and families in a caring atmosphere.
  • Screen, examine, and evaluate patients to establish diagnoses and treatment plans.
  • Develop treatment goals and methods in collaboration with patients and caregivers.
  • Implement physical therapy treatment plans and complete related documentation.
  • Design and apply adaptive devices, training patients in their use.
  • Select activities suited to the individual's capacity and interests.
  • Instruct patients in mobility activities and evaluate their progress.
  • Perform routine reexaminations to modify treatment plans as needed.
  • Incorporate health and wellness programming into treatment plans.
  • Conduct home and community mobility assessments and provide recommendations.
  • Perform other related duties as required.

Requirements

  • Valid Physical Therapy license in the state(s) of practice or proof of license eligibility.
  • Master's degree in Physical Therapy or a related field, or a Bachelor's degree in Physical Therapy with 5 years of progressively responsible experience.
  • Evaluation of foreign Physical Therapy degrees to ensure U.S. equivalency if applicable.
  • Valid driver's license or reliable transportation to patients' residences.
