About the position

The Physical Therapist will work in an outpatient setting, responsible for creating and managing individualized interdisciplinary care plans under physicians' orders. This role involves evaluating patients, developing treatment plans, setting functional goals, conducting follow-up assessments, and educating patients and their families. The therapist will also coordinate care with other medical and rehabilitation staff, maintain electronic medical records, and ensure compliance with relevant standards and regulations.

Responsibilities

• Establish and maintain comprehensive, individualized interdisciplinary plans of care under Physicians' orders.
• Perform evaluations and integrative treatment planning for patients.
• Set patient-specific functional goals and conduct follow-up assessments.
• Provide ongoing education to patients and their families.
• Coordinate the patient's Plan of Care with other Medical and Rehabilitation Staff.
• Maintain EMR in accordance with departmental policies and procedures, JCAHO standards, and regulations.
• Foster a constructive and supportive approach to department operations.
• Participate in process improvement and quality improvement measures.
• Provide supervision and support to Physical Therapy Assistants and Rehabilitation Technicians.
• Act as a clinical educator for students and other clinical staff.

Requirements

• Graduation from an accredited program of physical therapy or foreign-trained physical therapy with State Board Licensure.
• Current California Board of Physical Therapy licensure.
• BLS certification.
