About The Position
The Physical Therapist will work in an outpatient setting, responsible for creating and managing individualized interdisciplinary care plans under physicians' orders. This role involves evaluating patients, developing treatment plans, setting functional goals, conducting follow-up assessments, and educating patients and their families. The therapist will also coordinate care with other medical and rehabilitation staff, maintain electronic medical records, and ensure compliance with relevant standards and regulations.
