About the position

The Per Diem Physical Therapist is a healthcare professional responsible for providing therapy services on an as-needed basis, primarily to cover staff shortages, vacations, and leaves of absence. This role requires a forward-thinking individual who excels in assessing patients' strengths and weaknesses and creating effective treatment plans. The therapist must maintain patient confidentiality and foster a positive, high-energy environment to encourage patients in their healing and recovery process.

Responsibilities

  • Collect and record patient care data following guidelines of patient care service, regulatory agencies, and third-party payers.
  • Prepare for evaluations by obtaining patient history and demonstrating knowledge of patient diagnoses.
  • Select and administer tests and measures to assess patient conditions.
  • Synthesize evaluation results to formulate diagnosis, goals, prognosis, and plan of care.
  • Establish treatment plans based on evaluation results and best practices.
  • Collaborate with interdisciplinary teams, patients, and their support systems.
  • Assess and re-assess the need for modifications to the plan of care and goals.
  • Identify the need for consultations and refer patients to other healthcare professionals or services.
  • Initiate and complete discharge plans for patients.
  • Provide education and training to patients and their support systems.
  • Conduct functional training including ADLs/IADLs and therapeutic activities.
  • Manage treatment plans and exercise prescriptions for patient caseloads.
  • Provide safe and effective interventions in compliance with JAG standards.
  • Document treatment data and note writing using Prompt EMR for all treatments provided.

Requirements

  • Licensed Physical Therapist with relevant qualifications and training.
  • Strong clinical knowledge and ability to assess patient conditions effectively.
  • Experience in creating and implementing treatment plans.
  • Ability to collaborate with interdisciplinary teams and communicate effectively with patients and their support systems.
  • Knowledge of regulatory guidelines and reimbursement systems.

Nice-to-haves

  • Experience with electronic medical records (EMR) systems, particularly Prompt EMR.
  • Familiarity with JAG standards and billing practices.
  • Previous experience in a per diem or flexible role.
