About the position

The Per Diem Physical Therapist is a healthcare professional responsible for providing therapy services on an as-needed basis, primarily to cover staff shortages, vacations, and leaves of absence. This role requires a forward-thinking individual who excels in assessing patients' strengths and weaknesses, devising effective treatment plans, and maintaining a positive and encouraging environment for patients. The therapist will utilize their training and skills to aid patients in healing, recovering, and maximizing functional independence.

Responsibilities

• Collect and record patient care data following guidelines of patient care service and regulatory agencies.
• Conduct initial evaluations, daily notes, reassessments, progress reports, and discharge plans.
• Communicate effectively with team members regarding patient care.
• Prepare for evaluations by obtaining patient history and understanding diagnoses.
• Select and administer appropriate tests and measures for evaluations.
• Synthesize evaluation results to formulate diagnoses, goals, prognosis, and plans of care.
• Establish treatment plans based on evaluations and best practices.
• Collaborate with interdisciplinary teams and patients' support systems.
• Assess and modify plans of care and goals as needed.
• Identify the need for consultations and refer patients to other healthcare professionals.
• Initiate and complete discharge plans.
• Provide education and training to patients and their support systems.
• Conduct functional training including ADLs/IADLs and therapeutic activities.
• Manage treatment plans and exercise prescriptions for patient caseloads.
• Provide safe and effective interventions in compliance with JAG standards.
• Document treatment data and note writing using Prompt EMR for all treatments provided.

Requirements

• Licensed Physical Therapist with relevant clinical experience.
• Strong assessment and evaluation skills.
• Ability to develop and implement effective treatment plans.
• Excellent communication and interpersonal skills.
• Knowledge of patient care guidelines and regulatory requirements.

Nice-to-haves

• Experience with electronic medical records (EMR) systems.
• Familiarity with billing and coding practices in healthcare.
