About the position
We are seeking a dedicated and skilled Physical Therapist for our Home Health division to provide high-quality, patient-centered care in the home setting. This role is essential in fostering a positive rehabilitative environment for patients recovering from illness, injury, or surgeries. The successful candidate will possess a strong understanding of physical therapy practices and demonstrate the ability to adapt therapies to suit the home environment, making a meaningful impact in the lives of our patients.
Responsibilities
- Evaluate patients through physical assessments and medical history
- Develop personalized treatment plans based on individual needs and goals
- Implement therapeutic exercises and training programs
- Educate patients and their families on injury prevention and wellness
- Monitor and document patients' progress toward rehabilitation goals
- Collaborate with other healthcare providers to ensure holistic patient care
- Maintain accurate patient records and documentation in compliance with regulations
Requirements
- Master's degree in physical therapy
- 1 year of home health experience as a physical therapist
- Currently licensed to practice as a physical therapist within the state of agency operation
- Must possess and maintain valid CPR certification while employed in a clinical role
- Must be a licensed driver who can travel to all business locations
