Responsibilities

• Evaluate homebound patients referred for physical therapy.
• Develop and implement treatment plans tailored to individual patient needs.

• Assess patients' strength, motor control, joint mobility, and other functional abilities.
• Deliver and delegate physical therapy services as appropriate.

Requirements

• PT experience required.
• Graduate of an accredited Physical Therapy Program (BS or entry-level master's degree).
• Active Connecticut Physical Therapist license.
• CPR certification required at the time of start.
