About the position

The Physical Therapist (PT) at the Center for Living & Rehab plays a crucial role in the rehabilitation department, focusing on enhancing the physical well-being and functional recovery of residents in a skilled nursing facility. This position offers various employment types including full-time, part-time, and per diem, and emphasizes a supportive and community-driven work environment.

Responsibilities

  • Conduct comprehensive physical assessments to determine residents' physical impairments and functional limitations, focusing on mobility, balance, and gait.
  • Develop individualized treatment plans based on assessments, with clear goals for improving residents' mobility and reducing pain.
  • Accurately and consistently collect Section GG data as required for Annual, Quarterly, Falls, and Significant Change assessments.
  • Administer physical therapy interventions, including exercises, manual techniques, and modalities, to enhance residents' physical function.
  • Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress.
  • Conduct remote check-in through remote therapeutic monitoring (RTM) with discharged patients to home or next level of care.
  • Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMO/Manage care progress updates in EMRs (NetHealth/PCC).
  • Collaborate with the rehab team, including Occupational Therapists and Speech-Language Pathologists, to ensure comprehensive care for residents.

Requirements

  • Licensed Physical Therapist (PT) with relevant experience in a skilled nursing or rehabilitation setting.
  • Strong assessment and evaluation skills to determine residents' needs.
  • Ability to develop and implement individualized care plans.
  • Knowledge of Section GG data collection and documentation processes.
  • Experience with Point of Service (POS) care delivery and electronic medical records (EMRs).
  • Excellent communication skills for educating residents and collaborating with interdisciplinary teams.

Nice-to-haves

  • Experience with remote therapeutic monitoring (RTM).
  • Familiarity with various physical therapy modalities and techniques.
