About the position

Angels of Care Pediatric Home Health is seeking dedicated Physical Therapists (PT) to provide high-quality care to pediatric patients with various disabilities. The role involves implementing treatment programs in home and community settings, focusing on enhancing the physical, neurological, cognitive, and social/emotional functioning of children. Responsibilities

• Perform evaluations and interpret assessment results to develop appropriate goals for each child.
• Create and implement physical therapy treatment plans in collaboration with physicians.
• Assist pediatric patients in developing or regaining physical, neurological, cognitive, and social/emotional functioning.
• Educate and empower caregivers and families to integrate therapeutic activities into daily routines.
• Coordinate with referral partners to provide services in accordance with physician orders.
• Design and instruct patients, families, and caregivers in home exercise programs to ensure continuity of care after discharge.
• Document patient care services and care coordination in an electronic medical record system.
• Maintain patient confidentiality and trust.

Requirements

• State license
• Current CPR certification
• A minimum of 1 year of experience preferred
