About the position

The Physical Therapist at JAG Physical Therapy is responsible for assessing patients' strengths and weaknesses to create effective treatment plans aimed at healing, recovery, and maximizing functional independence. This role requires maintaining patient confidentiality and fostering a positive, high-energy environment to encourage patient success. The therapist will implement treatment plans, document patient care data, and collaborate with an interdisciplinary team to ensure comprehensive care.

Responsibilities

  • Collect and record patient care data following regulatory guidelines and reimbursement systems.
  • Prepare for evaluations by obtaining patient history and understanding diagnoses.
  • Select and administer tests and measures to formulate diagnosis, goals, prognosis, and plan of care.
  • Establish treatment plans based on evaluation results and best practices.
  • Collaborate with interdisciplinary teams and patients' support systems.
  • Assess and modify plans of care and goals as needed.
  • Initiate and complete discharge plans, providing education to patients and their support systems.
  • Provide functional training, including ADLs/IADLs and therapeutic activities.
  • Oversee treatment plans and exercise prescriptions for patient caseloads.
  • Provide safe and effective interventions in compliance with JAG standards.
  • Document treatments using WEB PT EMR and ensure accurate billing.
  • Promote JAG in the community through lectures and presentations.

Requirements

  • Bachelor's or Master's Degree in Physical Therapy required; Doctorate preferred.
  • Licensed Physical Therapist in the practicing state.
  • Completed state-regulated CEU requirements.
  • Valid CPR License.
  • Experience with EMR software preferred.
  • Excellent oral and written communication skills.
  • Strong clinical decision-making skills.
  • Excellent work ethic and dedication to patient success.
  • Basic computer skills.
  • Ability to work as a team member and independently with minimal supervision.

Nice-to-haves

  • Experience with EMR software is preferred.
