About the position

Responsibilities
- Evaluates the patient's functional status and physical therapy needs for CMS documentation.
- Consults with the physician to develop the therapy plan of care.
- Provides services within the scope of practice as defined by state laws and in accordance with the plan of care.
- Observes and reports the patient's response to treatment and any changes in condition to the nurse supervisor or physician.
- Treats patients to relieve pain, develop and restore function, and maintain maximum functioning using evidence-based techniques.
- Instructs and informs the healthcare team, patient, and family/caregivers regarding the plan of care and progress toward goals.
- Visits patients within the service area of the agency and other nearby LHC agencies as needed.
Requirements
- Current Physical Therapy licensure in the state of Texas.
- Current CPR certification.
- Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
