About The Position

The Physical Therapist at JAG Physical Therapy is responsible for assessing patients' strengths and weaknesses, creating effective treatment plans, and providing high-quality care in a supportive environment. This role emphasizes patient confidentiality, positive encouragement, and the goal of maximizing functional independence for patients. The therapist will work closely with an interdisciplinary team to implement and adjust treatment plans based on patient progress.

Requirements

  • Bachelor's or Master's Degree in Physical Therapy required; Doctorate preferred.
  • Licensed Physical Therapist in the state of practice.
  • Completed state-regulated CEU requirements in appropriate state (NY, NJ, PA).
  • Valid CPR License.
  • Experience/knowledge with EMR software preferred.
  • Excellent oral and written communication skills.
  • Strong clinical decision-making skills.
  • Excellent work ethic and dedication to patient success.
  • Basic computer skills.
  • Ability to work as a team member and independently with minimal supervision.

Nice To Haves

  • Experience with EMR software is preferred.

Responsibilities

  • Collect and record patient care data following regulatory guidelines and reimbursement systems.
  • Prepare for evaluations by obtaining patient history and understanding diagnoses.
  • Select and administer tests and measures to formulate diagnosis, goals, prognosis, and plan of care.
  • Establish treatment plans based on evaluation results and best practices.
  • Collaborate with interdisciplinary teams, patients, and their support systems.
  • Assess and re-assess the need for modifications to the plan of care and goals.
  • Identify the need for consultations and refer patients to other healthcare professionals as necessary.
  • Initiate and complete discharge plans, providing education and training to patients and their support systems.
  • Provide functional training, including activities of daily living and therapeutic activities.
  • Administer safe and effective interventions in compliance with JAG standards.
  • Oversee support staff including PTA, ATC, and PT aides.
