About the position
The position involves providing high-quality, patient-centered primary care services to residents in skilled nursing facilities (SNFs). The provider will manage clinical care, prescribe medications, and ensure timely documentation of patient encounters. This role emphasizes meaningful connections with patients and staff, supported by a flexible work schedule and a culture of appreciation.
Responsibilities
- Rounding and managing clinical care of the population at assigned SNFs.
- Prescribing medications and treatments for residents.
- Providing comprehensive primary care services to patients in SNFs.
- Timely documentation of all patient care encounters in the electronic health record system.
- Available for phone/secure messaging consultations with facility nursing staff.
- Ensuring on-time attendance for scheduled shifts.
Requirements
- Medical degree and appropriate licensure to practice as a provider.
- Experience in primary care or similar clinical settings.
- Ability to use electronic health record systems, specifically ChartEasy™, PointClickCare, MatrixCare, or American HealthTech.
- Reliable transportation to travel to assigned facilities.
Nice-to-haves
- Experience with conversational and Dragon dictation software.
- Familiarity with skilled nursing facility operations.
