About the position

The Physician Assistant role at Theoria Medical involves providing high-quality, patient-centered care primarily in skilled nursing facilities (SNFs). The position emphasizes a supportive work environment with flexible schedules, no on-call duties, and a focus on meaningful patient-provider relationships. The role includes daily rounding, medication management, and timely documentation using advanced electronic health record systems.

Responsibilities

• Rounding and managing clinical care of the population at assigned SNFs.
• Prescribing medications and treatments for residents at SNFs.
• Providing comprehensive primary care services to patients in SNFs.
• Timely documentation of all patient care encounters.
• Available for phone/secure messaging consultations with facility nursing staff.
• Completing documentation in Theoria's proprietary EHR system, ChartEasy™.
• Ensuring on-time attendance for scheduled shifts.
• Maintaining reliable transportation.

Requirements

• Current Physician Assistant licensure in Minnesota.
• Experience in primary care or post-acute care settings.
• Strong communication skills for patient and staff interactions.
• Ability to work independently and manage time effectively.
• Familiarity with electronic health record systems.

Nice-to-haves

• Experience with PointClickCare, MatrixCare, or American HealthTech EHR systems.
• Previous experience in skilled nursing facilities.
