About the position

The position is for a healthcare provider responsible for managing clinical care in skilled nursing facilities (SNFs). The role emphasizes patient-centered care, timely documentation, and effective communication with nursing staff. The provider will work flexible 6-hour shifts, ensuring high-quality service while enjoying a supportive work environment with various benefits.

Responsibilities

• Rounding and managing clinical care of the population at assigned SNFs.
• Prescribing medications and treatments for residents in SNFs.
• Providing high-quality, patient-centered, comprehensive primary care services.
• Timely documentation of all patient care encounters in the electronic health record system.
• Available for phone/secure messaging consultations with facility nursing staff.
• Ensuring on-time attendance for scheduled shifts.

Requirements

• Medical licensure to practice as a healthcare provider.
• Experience in providing primary care services in a clinical setting.
• Ability to use electronic health record systems, preferably ChartEasy™, PointClickCare, MatrixCare, or American HealthTech.
• Reliable transportation to travel to assigned facilities.

Nice-to-haves

• Experience in skilled nursing facilities or post-acute care settings.
• Familiarity with telehealth practices and technologies.
