Theoria Medical is seeking a part-time Physician Assistant to provide high-quality, patient-centered care in skilled nursing facilities (SNFs). This role involves managing clinical care, prescribing medications, and ensuring timely documentation while fostering meaningful connections with patients and staff. Responsibilities

• Rounding and managing the clinical care of the population at the assigned SNF.
• Prescribing medications and treatments for residents residing at SNFs.
• Providing comprehensive primary care services to patients in SNFs.
• Timely documentation of all patient care encounters.
• Available for phone/secure messaging consultations with facility nursing staff.
• Completing documentation in Theoria's proprietary electronic health record (EHR) system, ChartEasy™.
• Ensuring on-time attendance for scheduled shifts.
• Maintaining reliable transportation.

Requirements

• Current licensure as a Physician Assistant in the state of practice.
• Experience in primary care or skilled nursing facilities preferred.
• Strong communication skills for patient and staff interactions.
• Ability to work independently and manage time effectively.

Nice-to-haves

• Experience with electronic health record systems such as PointClickCare, MatrixCare, or American HealthTech.
