About the position

The Hospital at Home (HAH) program at South Shore Hospital is seeking experienced providers to deliver high-quality care to patients transitioning from the hospital to their homes. This part-time position offers flexibility and the opportunity to work in a collaborative, innovative care model that emphasizes patient-centered care. Providers will engage in direct patient care, work with a multidisciplinary team, and have the option for hybrid in-person and remote work after initial training.

Responsibilities

  • Admitting patients to the Hospital at Home unit.
  • Providing direct patient care.
  • Working collaboratively with a multidisciplinary team.
  • Participating in Hospital at Home patient acquisition through partnership with Intake RNs.
  • Providing direct clinical care and medical decision-making for patients admitted on the HAH unit.
  • Developing appropriate plans of care and follow-up based on diagnostic assessments and physical examination findings.
  • Ordering medications and writing prescriptions according to policies and procedures.
  • Participating in daily HAH Interdisciplinary Rounds.
  • Providing oversight of HAH Advanced Practice Clinicians.
  • Performing virtual nighttime coverage as needed, on a rotating schedule.

Requirements

  • Medical Degree from an accredited institution (MD/DO).
  • Internship and Residency in Internal Medicine or Family Medicine from an accredited institution.
  • BC/BE in Internal Medicine or Family Medicine.
  • Unrestricted Physician License from the Board of Registration in Medicine.
  • Massachusetts Controlled Substance Registration and DEA Registration.
  • Active ACLS certification.
  • Fluency in English, both verbal and written.
  • Ability to learn how to use Epic EMR.
