About the position

The Physician Liaison plays a critical role in enhancing patient care services at Floyd Medical Center. Reporting to the Director of the Executive Vice President and Chief of Patient Care Services, this position is responsible for utilizing the nursing process to develop, implement, and evaluate outcomes for defined patient populations. The Physician Liaison employs a diverse set of skills to educate, consult, and assist in the management of patients, ensuring the highest quality of care and optimal outcomes in a cost-effective manner. This role requires collaboration with a multidisciplinary patient care team, which includes physicians, nurses, assistive staff, and various therapy disciplines, to identify opportunities for improving patient care and outcomes while also focusing on cost reduction based on current research findings. In addition to direct patient care responsibilities, the Physician Liaison is tasked with supporting the mission, vision, and values of Floyd Medical Center. This includes contributing to the overall objectives of the organization by providing clinical expertise in a defined area of specialty. The Physician Liaison coordinates and delivers direct patient care as needed, collaborates with healthcare team members to address complex patient needs, and actively participates in discharge planning. The role also involves frequent communication with physicians regarding patient progress and needs, as well as leading initiatives to anticipate and address barriers to discharge. Overall, the Physician Liaison is integral to ensuring that patient care is delivered effectively and efficiently, aligning with the goals of Floyd Medical Center.

Responsibilities

  • Provide clinical expertise in a defined area of specialty.
  • Coordinate and provide direct patient care as indicated by the clinical situation, collaborating with physician staff to promote wellness and prevent illness.
  • Respond to healthcare team members' requests for intervention in complex patient needs.
  • Revise and adjust care plans to meet individual patient needs related to specialty.
  • Involve patient and family unit in goal setting and communicate with them about the plan of care and discharge goals.
  • Participate in discharge planning.
  • Collaborate frequently with physicians regarding patient progress and needs.
  • Round with providers to discuss patient progression.
  • Collaborate frequently with Directors in the evaluation of supplies and equipment that support service line.
  • Collaborate with utilization managers to identify customers within service lines to assess needs, plan interventions, implement activities, and evaluate outcomes that exceed customer expectations.
  • Lead SNAP Huddles on each nursing unit to anticipate barriers to discharge and help remove them.
  • Escalate barriers to discharge as needed.
  • Hold the care team accountable for meeting the patient's anticipated date of discharge.
  • Perform other related duties as assigned.
  • Utilize and participate in research opportunities in defined area of specialty.

Requirements

  • Graduate of an accredited school of Registered Professional Nursing.
  • Current license to practice as a registered nurse in the State of Georgia.
  • At least ten years working in the care of assigned patient population.

Nice-to-haves
