About the position

The Physician role at Theoria Medical involves serving as a medical director for skilled nursing facilities (SNF) while providing comprehensive primary care services. This position emphasizes work-life balance with flexible scheduling, no on-call duties, and a supportive environment with NP/PA assistance. The physician will engage in administrative responsibilities, quality initiatives, and direct patient care, ensuring compliance with regulations and fostering meaningful connections with patients and staff.

Responsibilities

  • Serve as medical director for skilled nursing facilities (SNF) and participate in administrative and quality initiatives.
  • Attend and participate in monthly quality assurance and process improvement meetings.
  • Provide guidance to facility staff and review/revise clinical guidelines as necessary.
  • Ensure compliance with state and federal regulations and train facility staff.
  • Supervise facility clinical staff and collaborate with Nurse Practitioners and Physician Assistants for resident care.
  • Provide high-quality, patient-centered primary care services by rounding on patients in SNFs.
  • Diagnose and treat patients with acute illnesses and chronic diseases.
  • Complete provider documentation in Theoria's proprietary electronic health record (EHR) system, ChartEasy™.
  • Participate in Theoria's Chronic Care Management, Transitional Care, RPM, and other medical quality initiatives.

Requirements

  • Medical degree (MD or DO) and valid medical license in North Carolina.
  • Experience in a medical director role or similar leadership position in healthcare.
  • Strong knowledge of state and federal regulations related to skilled nursing facilities.
  • Ability to collaborate effectively with clinical staff and manage patient care.
  • Proficiency in using electronic health record (EHR) systems.

Nice-to-haves

  • Experience with PointClickCare, MatrixCare, or American HealthTech EHR systems.
  • Familiarity with chronic care management and transitional care programs.
