About the position

The Physician role at Theoria Medical is a part-time position focused on providing high-quality, patient-centered primary care services to residents in skilled nursing facilities. The position emphasizes work-life balance with flexible scheduling, no on-call duties, and no weekend requirements. Physicians will collaborate with Nurse Practitioners and Physician Assistants to manage resident care, ensuring comprehensive treatment for both acute and chronic conditions. The role also includes utilizing advanced electronic health record systems for documentation and participating in various medical quality initiatives.

Responsibilities

• Serve as the attending physician for residents at each facility.
• Collaborate with Nurse Practitioners and Physician Assistants for day-to-day resident care.
• Provide comprehensive primary care services by rounding on patients in skilled nursing facilities.
• Diagnose and treat patients with acute illnesses and chronic diseases.
• Complete provider documentation in Theoria's proprietary electronic health record (EHR) system, ChartEasy™.
• Utilize facility EHR systems such as PointClickCare, MatrixCare, and American HealthTech.
• Participate in Chronic Care Management, Transitional Care, RPM, and other medical quality initiatives.
• Supervise collaborating nurse practitioners and physician assistants.
• Utilize electronic/online medical tools for administrative functions.

Requirements

• Medical degree (MD or DO) with a valid medical license in Michigan.
• Experience in primary care or related field.
• Ability to work collaboratively with a healthcare team.
• Proficiency in using electronic health record systems.

Nice-to-haves

• Experience in skilled nursing facilities or post-acute care settings.
• Familiarity with chronic care management and transitional care programs.
