About the position

Theoria Medical is seeking a dedicated physician to provide high-quality, patient-centered primary care services in skilled nursing facilities. This role offers flexibility in scheduling, with no on-call, weekends, or overnight duties, allowing for a balanced work-life experience. Physicians will collaborate with Nurse Practitioners and Physician Assistants to manage resident care, ensuring comprehensive treatment for both acute and chronic conditions. The position emphasizes meaningful patient interactions and a supportive work environment, with opportunities for professional growth and development.

Responsibilities

• Serve as the attending physician for residents in skilled nursing facilities.
• Collaborate with Nurse Practitioners and Physician Assistants for day-to-day resident care.
• Provide comprehensive primary care services by rounding on patients.
• Diagnose and treat patients with acute illnesses and chronic diseases.
• Complete provider documentation in Theoria's proprietary electronic health record (EHR) system.
• Participate in Chronic Care Management, Transitional Care, and other medical quality initiatives.
• Supervise collaborating nurse practitioners and physician assistants.
• Utilize electronic medical tools and state/county medical systems for administrative functions.

Requirements

• Medical degree (MD or DO) and valid medical license.
• Experience in providing primary care services in a skilled nursing facility setting.
• Familiarity with electronic health record (EHR) systems, particularly PointClickCare, MatrixCare, and American HealthTech.
• Ability to collaborate effectively with a healthcare team.

Nice-to-haves

• Experience with chronic care management and transitional care programs.
• Familiarity with telemedicine practices.
