About the position

The Comprehensive Care Advanced Care Practitioner (ACP) at ArchWell Health plays a crucial role in delivering high-quality healthcare to senior members. This position involves conducting thorough assessments, addressing care gaps, and ensuring continuity of care through in-person and telehealth visits. The ACP will work closely with the clinical team to prioritize member needs and enhance their overall healthcare experience.

Responsibilities

  • Conduct Prospective Chart Prep for clinical diagnostic and quality opportunities of identified members.
  • Engage with center leadership and care team members to prioritize scheduling based on outstanding care gaps.
  • Complete encounters with identified members in-center or via telehealth.
  • Address historical and suspected conditions presented via point of care interface.
  • Address open HEDIS measures of Focus On Care, including performing point of care testing.
  • Interpret evidence-based practice screenings and point of care testing.
  • Document assessments and plans for chronic conditions in the EMR.
  • Refill routine medications per ArchWell Health's Standing Order, excluding controlled substances.
  • Coordinate follow-up care and maintain communication with the member's care team.
  • Identify urgent and emergent situations and intervene appropriately.
  • Educate members on disease processes, preventative screenings, and medication adherence.
  • Attend training sessions and center meetings, demonstrating proficiency in Clinical Documentation Improvement (CDI).
  • Complete and lock Encounter Notes timely according to ArchWell Health's standards.
  • Achieve above-average member experience metrics.

Requirements

  • Active NP or PA license or ability to obtain by start date, unencumbered and free of disciplinary actions.
  • Active ANCC, AANP, or PA national certification in relevant fields.
  • 1+ years of clinical experience in a relevant clinical setting.
  • Experience in gerontology, cardiology, internal medicine, or endocrinology.
  • Ability to travel within the license state if requested.

Nice-to-haves

  • Experience with Electronic Medical Records
  • Advanced knowledge of ICD-10 diagnosis codes
  • Medicare Advantage experience including knowledge of the HCC model and STARs measures
  • Sensitivity to the diverse needs of culturally diverse staff, patients, and communities.
