About the position

The Physician position at Theoria Medical is a full-time role focused on providing high-quality, patient-centered care in skilled nursing facilities (SNFs). The physician will serve as the medical director, overseeing clinical staff and participating in quality initiatives while also managing resident care. This role offers a flexible schedule, no on-call or weekend duties, and a supportive work environment with NP/PA assistance.

Responsibilities

  • Serve as medical director for skilled nursing facilities (SNF) and participate in administrative and quality initiatives.
  • Attend and participate in monthly quality assurance and process improvement meetings.
  • Provide guidance to facility staff and ensure compliance with state and federal regulations.
  • Supervise facility clinical staff and train them as necessary.
  • Collaborate with Nurse Practitioners and Physician Assistants to manage day-to-day resident care.
  • Diagnose and treat patients with acute illnesses and chronic diseases.
  • Complete provider documentation in Theoria's proprietary electronic health record (EHR) system, ChartEasy™.
  • Participate in Theoria's Chronic Care Management, Transitional Care, RPM, and other medical quality initiatives.

Requirements

  • Medical degree (MD or DO) and valid medical license.
  • Experience in primary care or geriatric medicine preferred.
  • Ability to supervise and collaborate with nurse practitioners and physician assistants.
  • Familiarity with electronic health record (EHR) systems.
