About the position

The Population Health Care Coordinator plays a crucial role in supporting JPC clinics by managing the overall needs of the patient population. This position involves identifying care opportunities, conducting patient outreach, and participating in performance and process improvement initiatives. The coordinator is responsible for recommending areas of opportunity that align with the JPC patient-centered medical home efforts, ensuring that clinics can effectively meet the needs of their patients. In this role, the coordinator enhances quality initiatives by assisting clinics in outreach efforts through various methods. They work closely with clinic staff to ensure the accurate and reliable deployment of processes and protocols designed to meet identified goals. Participation in performance process improvement initiatives is a key responsibility, which may include conducting chart reviews, special audits, report validation, and documenting performance gains. The coordinator monitors performance indicators and analyzes data patterns and trends to assist clinics in achieving their quality improvement goals. Collaboration with practice managers and external stakeholders is essential, as the coordinator assists in gathering and preparing supporting documentation and responds to changes and problems as they arise. A solid understanding of various quality programs impacting ambulatory practices, such as patient-centered medical home requirements, meaningful use, and HEDIS measures, is necessary for success in this role. Additionally, the coordinator assists with the implementation of processes to meet patient-centered medical home requirements and ensures ongoing compliance within clinics. They recommend new reporting needs as necessary and utilize available electronic tools, such as EMR, population health databases, and HIEs, to complete quality and performance improvement initiatives. The coordinator also identifies and assists in developing educational opportunities for practice staff to enhance their responsibilities and improve the patient experience. Maintaining up-to-date reference materials and files related to health plan requirements is also part of the job.

Responsibilities

  • Enhances quality initiatives by assisting clinics in care opportunity outreach through a variety of methods.

  • Participates in the clinics' performance process improvement initiatives, conducting chart reviews, special audits, report validation, and documenting performance gains.
  • Monitors performance indicators and analyzes data patterns/trends to assist clinics in achieving quality improvement goals.
  • Collaborates with practice managers and external stakeholders, assisting managers in gathering and preparing supporting documentation, and responding to changes and problems as they arise.

Requirements

  • Licensed Practical Nurse
  • High School Graduate
  • Basic Cardiac Life Support (BLS or BCLS) certification from the American Heart Association (AHA)
