About the position

Mindoula Health is seeking a dedicated Population Health Case Manager to support SMI/SUD members in Louisiana. This role is crucial in delivering personalized case management within the Population Health Program, empowering members to improve their healthcare outcomes. The Case Manager will work closely with community members to address their behavioral, medical, and social needs, ensuring they actively participate in their healthcare journey.

Responsibilities

• Deliver personalized case management to SMI/SUD members.
• Work closely with community members to understand their health conditions and social needs.
• Empower members to improve their healthcare through targeted support.
• Collaborate with health plan partners to achieve clinical and financial outcomes.
• Maintain rigorous client record-keeping and data entry standards.
• Uphold professional standards as a representative of Mindoula Health.

Requirements

• Bachelor's Degree required.
• Two years of related experience in case management or a similar field.
• Knowledge of resources, social determinants of health, mental health, and substance abuse disorders.
• Excellent computer skills, particularly with Google Suite and Microsoft Office (Excel).
• Strong data entry skills and attention to detail.
• Excellent oral and written communication skills.
• Ability to work in a fast-paced environment with a strong sense of urgency.
