About the position

The Post Transplant Nurse Coordinator Lead for the heart transplant and VAD program is responsible for coordinating activities and overseeing the daily operations of the clinical care team. This role serves as a professional role model and leader, focusing on establishing evidence-based standards of care for patients undergoing heart transplant and ventricular assist device (VAD) procedures. The coordinator is directly involved in the interdisciplinary team approach for evaluation, treatment, and follow-up care of patients.

Responsibilities

  • Coordinate the clinical management of patient care under the supervision of the cardio-thoracic surgeon and transplant cardiologist.
  • Provide training and support for new heart transplant and VAD coordinators and other support staff.
  • Ensure open and clear communication with patients, their families, and healthcare team members.
  • Educate patients and families regarding VAD and transplant processes.
  • Develop, plan, conduct, and evaluate educational activities for patients, families, professionals, and the public.
  • Engage with medical facilities, referral sources, and healthcare team members to encourage participation in the transplant and VAD process.
  • Develop and oversee the Quality Assurance and Performance Improvement (QAPI) program.
  • Lead QAPI planning, morbidity and mortality reviews, and best practice protocol development.
  • Respond to calls 24 hours a day for patient care coordination.
  • Assist with the surgical implantation and explanation of VADs.
  • Round daily on inpatients with VADs and manage comprehensive clinical care throughout hospitalization.
  • Supervise professional and nursing support staff involved in patient care.
  • Develop, collect, record, and maintain UNOS and VAD data registries.

Requirements

  • Bachelor's Degree in Nursing from an accredited School of Nursing.
  • 3 or more years of heart transplant/VAD experience.
  • Licensed Registered Nurse (LRN) - Single State - State Board of Nursing with a current active multistate RN license in good standing.

Nice-to-haves

  • Master's Degree in Nursing.
