About the position

The Postpartum Maternal/Child Health Care Coordinator at Community of Hope is responsible for providing education and support to patients in postpartum group care. This role focuses on enhancing postpartum health and wellbeing, infant care, and parenting through group education sessions. The coordinator will work closely with patients to set and achieve health-related goals, facilitate educational groups, and connect patients with necessary medical or social supports. This full-time position involves both onsite and remote work, emphasizing a commitment to improving health outcomes in under-resourced communities in Washington, DC.

Responsibilities

  • Provides education and assists patients in developing short- and long-term goals focused on postpartum self-management, infant care, and parenting.
  • Acts as a liaison between patients, caregivers, providers, clinical staff, and program leadership.
  • Facilitates at least six infant massage parent education groups annually using the Infant Massage USA model.
  • Supports registration for breastfeeding/chest feeding groups and distributes incentives after each group.
  • Co-facilitates multiple postpartum group care appointments with a midwife or FNP weekly to monthly.
  • Helps develop and maintain accurate postpartum group calendars and communicates changes to relevant parties.
  • Assesses barriers to adherence to group schedules and maintains contact with postpartum patients regarding missed appointments.
  • Actively recruits postpartum group participants by connecting with patients before the conclusion of each CenteringPregnancy group.
  • Documents all patient encounters and group materials in the electronic record.
  • Represents Community of Hope at health-related community functions as requested.

Requirements

  • High School Diploma/GED required; Bachelor's degree in public/community health education, maternal & child health, or social work preferred.
  • 2 years of practical experience related to maternal and child health, doula and/or childbirth education, home visiting, or related field.
  • Experience providing education and coordinating health programs in a group or individual setting, specifically in maternal & child health.
  • Ability to express enthusiasm about breastfeeding and support lactating individuals.
  • Knowledge of the CenteringPregnancy and/or Parenting model preferred.
  • Commitment to work with underserved populations and ability to work in non-profit environments.
  • Demonstrates cultural competence in communicating with low-income populations.
  • Ability to travel between COH sites or relocate to a different COH site as needed.
  • Ability to work Saturdays and evenings.
  • Strong interpersonal skills and ability to be creative, innovative, and multi-task.
