About the position

The PACT Licensed Practical Nurse (LPN) at the Department of Veterans Affairs is responsible for providing specialized nursing care to adult and geriatric patients with both acute and chronic health issues. The LPN collaborates with the Patient Aligned Care Team (PACT) to develop individualized health care and management plans for patients, ensuring comprehensive care delivery in a primary care setting.

Responsibilities

• Administer nursing care to patients in a primary care setting.
• Document nursing care and gather data for assessments on chronic diseases and preventive screenings.
• Administer medications and observe for therapeutic effects and adverse reactions.
• Take and document vital signs, height, and weight according to policy.
• Recognize urgent patient care situations and initiate appropriate emergency interventions.
• Perform EKGs and notify the RN/APN/MD of findings.
• Use motivational interviewing and health coaching to guide patients in self-management.
• Inventory supplies and medications, ensuring stock levels are appropriate.
• Complete clinical reminders and appropriate consults as indicated.
• Maintain confidentiality of patient information and demonstrate professional behavior.

Requirements

• U.S. Citizenship is required.
• Graduation from an accredited school of practical or vocational nursing.
• Full, active, current, and unrestricted licensure as a practical or vocational nurse in a U.S. state or territory.
• At least one year of qualifying experience at the GS-5 level or equivalent.
