About the position

As a Practice Nurse (LPN) at New Holland Family Medicine, you will provide comprehensive nursing services to patients of all ages. Your role will involve offering clinical advice, administering medications, and assisting in various medical procedures. You will also focus on patient education regarding preventive care and chronic disease management, while coordinating care and referrals to enhance the overall patient experience and quality of care.

Responsibilities

  • Provide clinical advice by phone/electronic correspondence based on clinical judgments and/or triage protocols.
  • Consult with providers as necessary.
  • Educate patients on preventive care, chronic disease management, and medications.
  • Prepare patients for visits using appropriate rooming standards.
  • Administer injections and medications as prescribed by the provider.
  • Assist providers with various procedures, minor surgeries, and exams.
  • Perform Point of Care Testing, including hearing, flu, urine, and strep tests.
  • Collect blood or other laboratory specimens and prepare them for testing.
  • Authorize prescription refills as instructed by the provider.
  • Assist with test tracking and monitoring overdue results.
  • Remove sutures and change dressings on wounds as necessary.
  • Ensure exam rooms are adequately supplied and cleaned after each visit.
  • Maintain and clean medical instruments and equipment after procedures.
  • Assist providers and patients in reviewing and revising care plans and assessing patient progress.
  • Follow up with patients between visits as necessary.
  • Assist with proactive outreach for patients requiring preventive and chronic disease services.
  • Perform care coordination activities among specialists and transitions of care.
  • Assist in coordination of referrals, education, and community resources.
  • Participate in performance improvement initiatives to enhance patient experience and quality of care.
  • Participate in daily huddles with the provider.
  • Serve as a patient experience advocate during interactions with patients, visitors, and colleagues.
  • Communicate professionally with all patients, visitors, and colleagues, prioritizing customer service.
  • Handle patient information confidentially according to privacy policies.

Requirements

  • Completion of a LPN or RN educational program.
  • Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) issued by the PA State Board of Nursing.
  • AHA CPR/BLS certification required within 30 days of hire.
  • Ability to travel to multiple sites as required.

Nice-to-haves

  • One (1) to Two (2) years of nursing experience in a primary or specialty care physician's office.
  • Prior triage, emergency room, cardiac care, intensive care, or other related specialty nursing experience.
