About the position

The Community Health PrEP Navigator plays a crucial role in supporting individuals at high risk for HIV by providing education, guidance, and assistance in accessing PrEP (Pre-exposure Prophylaxis) services. This position is designed to engage clients through follow-up communications, both by phone and in person, to ensure they are informed about and can access necessary health services. The Navigator will conduct educational sessions on HIV, STIs, and general health, tailored to the needs of specific populations, and will work closely with various departments within Whitman-Walker Health to facilitate comprehensive care. In this role, the PrEP Navigator will be responsible for community outreach, collaborating with other agency navigators to identify and engage new clients in PrEP services. They will educate patients about PrEP, assist with appointment scheduling, and provide support during initial visits. The Navigator will also conduct HIV and STD testing, offer counseling, and help clients navigate barriers to accessing healthcare. Documentation of all activities is essential, as is participation in quality improvement initiatives. The position requires a commitment to the mission and values of Whitman-Walker Health, including respect, dignity, and cultural sensitivity. The Navigator will be expected to maintain data integrity, adhere to HIPAA regulations, and demonstrate excellent customer service. Flexibility in working hours, including evenings and weekends, may be necessary to meet the needs of the community and the organization.

Responsibilities

  • Provide support, education, and guidance to clients at high risk for HIV regarding PrEP services.
  • Conduct community outreach to engage new consumers in PrEP services.
  • Facilitate educational sessions on HIV, STIs, and general health for targeted populations.
  • Assist patients in scheduling and attending their first PrEP appointments.
  • Conduct HIV and STD testing and provide prevention counseling.
  • Document all work activities and assist in the creation of reports as necessary.
  • Work with various departments to develop data collection protocols for PrEP referrals.
  • Assist patients with insurance navigation and financial assistance for PrEP medications.
  • Participate in organizational quality and performance improvement activities.
  • Provide adherence follow-ups to ensure patient retention in PrEP care.

Requirements

  • Bachelor's Degree or at least four years of direct experience in a related field.
  • Written and oral fluency in Spanish or Amharic is required.
  • Knowledge of ambulatory healthcare, PEP/PrEP, HIV, and STIs.
  • Experience working with MSM of color and transgender individuals.
  • Excellent oral and written communication skills.
  • Ability to conduct sensitive and empathetic interviews with clients.
  • Knowledge of HIV/STD risk factors and counseling techniques.
  • Ability to work independently with minimal supervision.

Nice-to-haves

  • HIV testing and training certification from INSTI preferred.
  • Experience providing counseling to people living with HIV preferred.
  • Completion of phlebotomy training is strongly preferred.
