About the position
The Preservice Review Nurse position at UnitedHealthcare involves providing individualized care strategies for patients in Atlantic and Cumberland Counties, NJ. The nurse will assess, plan, and implement care while advocating for patients and ensuring coordinated care among stakeholders.
Responsibilities
- Assess, plan, and implement individualized care strategies for patients.
- Identify and initiate referrals for social service programs, including financial and psychosocial support.
- Manage the care plan through the continuum of care as a single point of contact.
- Communicate health-related information to stakeholders to ensure quality care.
- Advocate for patients and families to represent their needs and choices.
Requirements
- Current, unrestricted independent licensure as a Registered Nurse in New Jersey.
- 2+ years of clinical experience.
Nice-to-haves
- Background in managing populations with complex medical or behavioral needs.
- Experience with electronic charting.
- Experience with arranging community resources.
- Field-based work experience.
- Managed Long Term Services and Supports (MLTSS) or Aged, Blind, and Disabled Programs (ABD) experience.
