5 Star Northwest Medical Homes, previously known as Springfield Family Physicians, is seeking an energetic and hardworking Primary Care Behavioral Health Consultant (IBHC). This role is pivotal in the diagnosis, treatment, and maintenance of a diverse behavioral health and medical population, utilizing the primary care behavioral health model of care. The IBHC will work closely with primary care providers and other team members to ensure comprehensive care for patients. The IBHC will perform psychosocial assessments, develop and implement individualized plans of care, and assess crisis situations to provide necessary resources or referrals. This position requires collaboration with patients, their support systems, healthcare providers, insurers, and community resources. A typical clinic day involves alternating 30-minute follow-up appointments with 30-minute warm hand-off visits to optimize same-day access to care. In our larger clinic, two IBHCs will balance this schedule to maximize patient access. Key functions include providing consultation to medical providers on behavioral health management, conducting brief screenings for behavioral health needs, and offering short-term interventions to motivate patients in managing their health care. The IBHC will also conduct group visits, participate in ongoing training, and maintain a current listing of community resources to support the target population. Accurate documentation in patient medical records and participation in multidisciplinary case conferences are essential components of this role. The IBHC will also engage in population health projects and maintain professional knowledge through continuous education and networking. This position is integral to providing an excellent service experience and adhering to established policies and standards of conduct. The IBHC will also be responsible for identifying areas for improvement and development within the practice.
