Responsibilities

• Provide patient-centered care and support to individuals across the lifespan.
• Triage acute illnesses and manage chronic conditions effectively.

• Collaborate with primary care providers, medical assistants, and other healthcare professionals.

Requirements

• Current New Hampshire Nursing License
• Experience in primary care and ambulatory health care preferred
• Current BLS certification
• 2-3 years of experience in primary care
