About the position

ChenMed is a physician-led and mission-driven primary care organization that is rapidly expanding and is currently one of the most successful full-risk Medicare Advantage providers in the nation. Our mission is to honor seniors with affordable VIP care that delivers better health. We are looking for a Primary Care Physician (PCP) who is innovative, entrepreneurial-minded, and possesses a strong work ethic and ambition. The PCP will be responsible for providing care to a panel of up to 450 patients, demonstrating accountability for outcomes, and building trusting relationships with patients to influence their health positively. The PCP acts as a health coach, helping patients set and achieve health goals, and simplifies processes to make behavior change actionable. The PCP will independently provide care for patients with acute and chronic illnesses, taking full accountability for patient care and outcomes. They will engage with hospitalists when their patients are hospitalized and will lead a care team that includes a medical assistant, care facilitator, and care coordinator. The PCP will also participate in risk and quality management programs and adhere to departmental goals, standards of performance, and regulatory compliance. The role requires a commitment to high-quality care and continuous improvement in practice and operations. In addition to clinical responsibilities, the PCP will play a vital role in fostering a positive culture within the medical center, championing love, accountability, and passion. They will also be involved in recruiting patients and additional providers for the center. The ideal candidate will be proficient in evidence-based medicine, possess strong leadership skills, and prioritize self-care to ensure sustainability in their practice. Fluency in English is required, and additional language skills may be beneficial depending on the center's needs.

Responsibilities

  • Provide care for patients with acute and chronic illnesses.
  • Demonstrate accountability for patient care and outcomes.
  • Act as a health coach to help patients set and achieve health goals.

  • Lead a care team consisting of a medical assistant, care facilitator, and care coordinator.

  • Recruit patients and additional providers for the center.

Requirements

  • MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required.
  • Must be able to obtain a State Medical License or already have a current, active State Medical License for the state(s) in which he/she will be working.
  • Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred.
  • Basic Life Support (BLS) certification required.
  • Must have a current DEA number for schedule II-V controlled substances.
