About the position

The Primary Care Physician will join the South County Medical Group in Westerly, Rhode Island, providing patient-centered care within a collaborative environment. The role emphasizes coordination with specialists and support staff to deliver comprehensive healthcare services, including on-site diagnostics and treatments, while ensuring a seamless transition of care for patients.

Responsibilities

• Provide primary care services to patients in a patient-centered medical home model.
• Collaborate with pharmacists and nurse case managers to support high-risk patient populations.
• Coordinate care with specialists within the South County Medical Group.
• Perform various medical procedures including suturing and administering treatments.
• Utilize on-site diagnostic services such as x-ray, ultrasound, and lab draws.
• Participate in after-hours care through the adjacent Express Care facility.

Requirements

• Must be licensed to practice in Rhode Island.
• Board Certified or Board Eligible in Family Medicine or Internal Medicine.
• Credentialing by South County Health Medical Staff Office is required.
