About the position
The Licensed Practical Nurse (LPN/LVN) is responsible for providing skilled nursing care under the supervision of a Registered Nurse, following care plans and physician orders for individual patients. This role emphasizes adherence to confidentiality standards and professional boundaries while delivering quality patient care.
Responsibilities
- Responsible for the delivery and coordination of quality patient care in compliance with physician orders.
- Continuously observes and assesses patient condition and care needs, reporting changes to the supervisor and/or physician.
- Documents all activities, assessments, nursing actions, responses, and coordination of care in a timely manner.
- Participates in, implements, and updates the nursing care plan.
- Takes appropriate nursing action based on assessment to achieve expected outcomes.
- Recognizes changes in patient needs and implements care to prevent or reduce risk.
- Complies with Aveanna policies, state and federal regulations, and the Nurse Practice Act.
- Provides care utilizing infection control measures according to OSHA standards.
- Educates patients and families regarding disease processes, self-care techniques, and prevention strategies.
- Maintains knowledge of nursing competencies through educational programs and skill evaluations.
Requirements
- Graduate of an accredited school of nursing.
- Current, unrestricted state license as a Licensed Nurse in the state of practice.
- Current CPR certification.
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care policies.
Nice-to-haves
- Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
- Home health experience.
