About the position

The Bilingual Mobile Outreach Social Worker (MSW) at Empath Health is a part-time role focused on community outreach and support for individuals and families. The position involves coordinating service inquiries, providing access to resources, and ensuring excellent customer service through compassionate and empathetic interactions. The social worker will engage with patients, families, and community partners to assess needs, facilitate care navigation, and promote the well-being of the community served.

Responsibilities

• Provide optimum access to Empath Health Care Continuum via various inquiry methods.
• Articulate information regarding programs and services in a customized manner.
• Work effectively with patients, families, and community providers to ensure accurate referrals.
• Ensure physician certification and pre-admission regulatory requirements are met and documented in EMR.
• Assess patient/family needs for case management assistance and link them to appropriate resources.
• Provide information about financial resources including Medicare and Medicaid.
• Participate in interdisciplinary team meetings to discuss psychosocial aspects of care.
• Perform initial patient/family contact to determine eligibility for programs or services.
• Complete psychosocial assessments to identify needs and assist in care planning.
• Maintain confidentiality and provide a supportive atmosphere for individuals seeking assistance.
• Build rapport with community partners and understand their perspectives and needs.
• Anticipate and resolve complaints effectively, using feedback as a learning opportunity.
• Assist with care coordination and update medical records with current financial status and care level.
• Educate and support patients/families in decision-making processes.
• Provide community resource information as needed.
• Collaborate with Care Navigation nurse to assist patients/families in program selection.
• Act as an ambassador for Empath Health's Network of Care and utilize resources efficiently.
• Promote the organization's vision, mission, and values, contributing to team development.
• Update knowledge of Empath Health's services and participate in educational offerings.
• Develop and implement projects to improve cultural competence within the organization.
• Engage in community outreach to increase visibility of services and opportunities.

Requirements

• Master of Social Work (MSW) degree from an accredited school of Social Work.
• At least one year of social work or counseling experience in a healthcare setting.
• Bilingual - fluent in English and Spanish.
• Valid Florida State Driver's License.
• Reliable transportation for field-based tasks.
• Proof of valid automobile insurance.

Nice-to-haves
