About the position

As a PRN Case Manager RN at Menorah Medical Center, you will play a crucial role in ensuring high-quality, patient-centered care for rehabilitative patients. This position allows you to make a significant impact on patient outcomes by managing and coordinating care tailored to the specific needs of each patient. You will work closely with the Rehabilitation team, collaborating with the Rehab Program Director and Facility Case Management Director to enhance departmental functions and improve patient care delivery. Your responsibilities will include providing program orientation to patients, families, and caregivers, which encompasses explaining the case manager's role, the philosophy of rehabilitation, and the criteria for continued stay and discharge. You will also educate patients and their families about Medicare and insurance benefits, grievance procedures, and the treatment plan process, ensuring they understand their rights and responsibilities. In this role, you will act as the primary coordinator for patient and family education, promoting active participation in discussions regarding treatment plans, goals, and patient status. You will facilitate Family Conferences and other interactions to ensure that the voices of patients and their families are heard and considered in the rehabilitation process. Additionally, you will be responsible for implementing treatment plans that align with the patient's strengths, needs, and preferences, ensuring that they are actively involved throughout their rehabilitation journey. Documentation is a key aspect of your role, as you will need to record findings from Discharge Planning Evaluations and psychosocial assessments, communicating any social, financial, or discharge needs to the relevant parties. Responsibilities

  • Provide program orientation to patients, families, and caregivers regarding the case manager's role and rehabilitation philosophy.
  • Act as the coordinator of patient/family/caregiver education, promoting their participation in team discussions.
  • Ensure the implementation of the patient's treatment plan that supports their strengths, abilities, needs, and preferences.
  • Document findings from Discharge Planning Evaluations and psychosocial assessments, communicating patient needs and preferences.
  • Promote consistent, positive patient interactions that enhance patient service.

Requirements

  • Registered Nurse (RN) license required.
  • Registered Nurse Diploma or Associate Degree in Nursing.
