About the position

The PRN Hospice RN at SSM Health is responsible for coordinating and performing interdisciplinary care plans for patients in various settings, including homes and assisted living facilities. This role involves assessing patient needs, collaborating with healthcare teams, and providing skilled nursing care under the direction of attending physicians. The position is designed for entry-level nurses, offering a flexible work schedule as needed.

Responsibilities

• Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
• Performs point of care testing according to policies and procedures.
• Collaborates and communicates with physicians regarding the plan of care and changes to patient status.
• Provides regular updates to physicians on patient progress and recommends revisions to care plans.
• Coordinates with multi-disciplinary team members and outside agencies to achieve expected outcomes for patient care.
• Completes admission paperwork and verifies insurance information.
• Conducts comprehensive medical assessments and enters data into clinical programs.
• Provides skilled nursing care, including taking vitals, wound care, IV care, and injections.
• Documents visit notes, treatments, and care accurately and timely.
• Responds to calls from patients and family members, addressing needs and concerns.

Requirements

• Graduate of an accredited school of nursing or equivalent education for licensing.
• Registered Nurse (RN) License in the relevant state.
• Basic Life Support (BLS) Certification.
• Driver's License.
