About the position

Citizens Memorial Hospital Foundation (CMH) is seeking a dedicated Nurse to join our team at Parkview Healthcare Facility, a skilled nursing facility located in Bolivar, MO. This 78-bed facility includes a specialized memory care unit designed for individuals suffering from Alzheimer's disease and dementia. As a Nurse at Parkview, you will have the unique opportunity to build meaningful relationships with our residents and collaborate with a supportive healthcare team. In this role, the licensed practical nurse (LPN) will play a crucial part in delivering high-quality care to our residents. You will utilize your strong critical thinking and assessment skills to ensure that care is provided in accordance with federal and state regulations. The Charge Nurse will assign care duties to nursing personnel based on their training and experience, ensuring that all residents receive the appropriate level of care. Your responsibilities will include safe medication administration, conducting thorough resident assessments, monitoring changes in Activities of Daily Living (ADLs), and administering treatments and procedures as per physician orders. Additionally, you will be an integral member of the interdisciplinary team, facilitating resident admissions and discharges, as well as contributing to the development of each resident's Plan of Care. This position is PRN, allowing for flexible scheduling to meet the needs of the facility and its residents. Responsibilities

• Provide excellent care to residents in accordance with federal and state regulations.
• Utilize strong critical thinking and assessment skills in daily nursing duties.
• Administer medications safely and effectively.
• Conduct resident assessments and monitor changes in Activities of Daily Living (ADLs).
• Administer treatments and procedures as per physician orders.
• Facilitate resident admissions and discharges as part of the interdisciplinary team.
• Develop and contribute to the resident Plan of Care.

Requirements

• Licensed Practical Nurse (LPN) certification required.
• Strong critical thinking and assessment skills.
• Experience in a skilled nursing or long-term care facility preferred.
• Ability to work collaboratively within a healthcare team.
