The PRN Occupational Therapist (OTR) position at Enhance Rehabilitation involves evaluating and treating patients in a skilled nursing facility (SNF) setting. The role emphasizes providing excellent patient care and customer service, collaborating with facility staff, and ensuring individualized treatment plans are developed and followed.

Responsibilities

• Evaluate and treat patients in a SNF setting following an individualized plan of care.
• Provide excellent care for patients.
• Deliver excellent customer service to patients and family members.
• Collaborate with facility staff to ensure excellent care for patients.
• Evaluate new patients, develop plans of care, and set measurable, functional goals.
• Treat patients, address their concerns, and educate them on their injury, progress, and probable outcomes.
• Re-evaluate patient progress, revise plans of care, and reset goals.
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges, and daily progress notes.
• Supervise Occupational Therapy Assistants.

Requirements

• Valid Occupational Therapist license in Illinois.
• Experience in a skilled nursing facility (SNF) setting is preferred.
• Strong communication and interpersonal skills.

Nice-to-haves

• Experience with electronic health records (EHR).
• Knowledge of current rehabilitation practices.
