About the position

As a Pharmacist at Kindred Transitional Care Hospitals, you will play a crucial role in the recovery process of chronic, critically ill, and medically complex patients. You will collaborate with an interdisciplinary team to develop individualized care plans, ensuring the safe and effective use of medications. This position emphasizes career growth through development programs and training, contributing to the overall goal of delivering intensive, multidisciplinary care to transition patients to lower levels of care safely and efficiently.

Responsibilities

• Collaborate with the Director of Pharmacy to compound and dispense medications and pharmaceutical supplies according to standard procedures.
• Participate in Performance Improvement plans within the hospital.
• Consult with and advise Administration, Medical Staff, and hospital staff on quality, regulatory, and risk issues.
• Support and serve as a consultant to all clinical and Medical Staff.
• Assist the Director in coordinating and implementing Pharmacy activities.
• Interpret medication orders and dispense medications in accordance with legal, ethical, and professional standards.
• Supervise technical support staff.
• Assist with clinical, quality management, and education programs.
• Provide support for all departments regarding patient care for ages 18 and older, including geriatric patients.
• Perform clinical duties such as renal dosing, drug regimen review, and monitoring of antibiotics and anticoagulants.

Requirements

• Graduate of an accredited School of Pharmacy with a Pharmacist degree.
• Current state license in good standing.
• Minimum of three (3) months of experience in hospital pharmacy.
• Pharm D or residency preferred.
• Working knowledge of OSHA, JCAHO, HCFA, and state guidelines.
• Experience in facilitating teams.
• Experience in writing policies and procedures.
