About the position

The Physical Therapist at Enhance Rehabilitation plays a crucial role in delivering high-quality care to patients in a Skilled Nursing Facility (SNF) setting. This position involves evaluating and treating patients based on individualized care plans, ensuring that each patient receives the best possible treatment and support throughout their rehabilitation journey.

Responsibilities

• Providing excellent care for patients
• Providing excellent customer service to patients and family members
• Collaborating with facility staff to ensure excellent care for patients
• Evaluating new patients and developing plans of care
• Setting measurable, functional goals for patient recovery
• Treating patients and addressing their concerns
• Educating patients on their injury, progress, and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress and revising plans of care
• Completing timely and accurate documentation including evaluations, re-evaluations, discharges, and daily progress notes.

Requirements

• Licensed Physical Therapist
• Experience in a Skilled Nursing Facility (SNF) setting
• Strong communication and interpersonal skills
• Ability to develop individualized care plans
• Knowledge of rehabilitation techniques and patient care standards

Nice-to-haves

• Experience supervising Physical Therapist Assistants
• Familiarity with electronic health record (EHR) systems
• Certification in specialized therapy techniques
