About the position

Enhance Rehabilitation is currently seeking qualified Physical Therapists to join our dynamic team in Marengo, IL. As a Physical Therapist at Enhance Rehabilitation, you will play a crucial role in providing high-quality care to patients in a Skilled Nursing Facility (SNF) setting. This position involves evaluating and treating patients based on individualized plans of care tailored to meet their specific needs. You will be responsible for ensuring that each patient receives the best possible care, which includes not only physical rehabilitation but also excellent customer service to both patients and their family members. In this role, you will collaborate closely with facility staff to ensure that all aspects of patient care are addressed effectively. Your responsibilities will include evaluating new patients, developing comprehensive plans of care, and setting measurable, functional goals that align with each patient's rehabilitation needs. You will also be tasked with treating patients, addressing their concerns, and educating them about their injuries, progress, and probable outcomes. Additionally, you will supervise Physical Therapist Assistants, ensuring that they are providing appropriate support and care to patients. A key aspect of your role will be to regularly re-evaluate patient progress, revise plans of care as necessary, and reset goals to reflect the ongoing needs of your patients. Timely and accurate documentation is essential, and you will be responsible for completing evaluations, re-evaluations, discharges, and daily progress notes to maintain comprehensive records of patient care.

Requirements

• Licensed Physical Therapist in the state of Illinois
• Experience in a Skilled Nursing Facility (SNF) setting preferred
• Strong communication and interpersonal skills
• Ability to work collaboratively with a team
• Commitment to providing high-quality patient care
• Proficient in documentation and patient record management.

Nice-to-haves

• Experience with geriatric patients
• Knowledge of rehabilitation techniques and modalities
• Familiarity with electronic health record (EHR) systems
