About the position

As a Hospice RN/RN Case Manager at Gentiva Hospice, you will play a crucial role in providing compassionate care to patients and their families during some of the most challenging times in their lives. Reporting directly to the Administrator, Executive Director, or Patient Care Manager Senior, you will maintain primary control and professional management of each patient, ensuring that their needs are met with dignity and respect. Your responsibilities will include acting as the primary liaison between the physician, patient/family, and the hospice team, facilitating communication and collaboration to provide the best possible care. In this position, you will provide direct patient care services to hospice patients, as well as supportive care to their families. You will manage a reasonable caseload, fostering trusting and compassionate relationships with your patients and their families. Empowering caregivers by educating them on how to care for their loved ones will be a key aspect of your role. You will also collaborate with an interdisciplinary team, which includes Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA), and support staff, to ensure that each patient receives comfort, peace, and dignity in their care. Your responsibilities will extend to evaluating patient and family needs, assisting in the preparation and execution of individualized care plans, and maintaining all assigned patient clinical records. Participation in company quality assessment and performance improvement programs will also be part of your duties, ensuring that the highest standards of care are upheld throughout the organization.

Responsibilities

  • Maintain primary control and professional management of each patient.
  • Act as primary liaison between physician, patient/family, and hospice team.
  • Provide direct patient care services to hospice patients and supportive care to the patient's family.
  • Empower patient's caregivers by educating them on care for their loved one.
  • Manage and collaborate with an interdisciplinary group consisting of LPNs, CNAs, and support staff.
  • Evaluate patient and family needs and assist in the preparation and execution of the plan of care for each patient.
  • Maintain all assigned patient clinical records.
  • Participate in company quality assessment and performance improvement programs.

Requirements

  • Compassionate, kind, and empathetic nature towards patients and their families.
  • Comfortable providing care in various settings, including the patient's home.
  • Demonstrate flexibility and composure while supporting others in a time of crisis.
  • RN license in the state of requested employment (or eligibility to obtain).
  • 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology strongly preferred.
  • Preferred experience includes hospital Staff RN, Med Surg, PACU, Float pool, ER, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, or travel nursing.
  • Current driver's license and automobile insurance.
  • Current CPR certification.

Nice-to-haves

  • Experience in hospice or palliative care settings.
  • Background in critical care or emergency nursing.
