Reporting directly to the Administrator, Executive Director, or Patient Care Manager Senior, you will maintain primary control and professional management of each patient, ensuring that their needs are met with dignity and respect. Your responsibilities will include acting as the primary liaison between the physician, patient/family, and the hospice team, facilitating communication and collaboration to provide the best possible care. In this position, you will provide direct patient care services to hospice patients, as well as supportive care to their families.
