As a Case Manager PRN at Overland Park Regional Medical Center, you will play a crucial role in ensuring high-quality, patient-centered care by managing rehabilitative care to achieve optimum outcomes for patients. Your responsibilities will include providing, coordinating, and directing care tailored to the specific needs of each rehabilitation patient. You will collaborate closely with the Rehab Program Director and Facility Case Management Director to ensure departmental functions are executed effectively. In this role, you will be responsible for orienting patients, families, and caregivers about the rehabilitation program, including the case manager's role, the philosophy of rehabilitation, and the criteria for continued stay and discharge. You will also educate patients and their families about Medicare and insurance benefits, grievance procedures, treatment plans, and their rights and responsibilities. Your role will involve acting as the coordinator for patient, family, and caregiver education, promoting their participation in discussions regarding plans, goals, and status through Family Conferences and other interactions. You will ensure the implementation of the patient's treatment plan, which supports their strengths, abilities, needs, and preferences, facilitating their involvement throughout the rehabilitation process. Additionally, you will document findings from the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating any social, financial, or discharge needs and preferences of the patient and their family. You will be accountable for promoting consistent, positive patient interactions that enhance the agenda of unparalleled patient service. The unit is characterized by amazing teamwork across all levels, and you will assist in educating patients and their family members, ensuring they have the necessary resources and tools for a successful discharge. The PRN position will support needs from Monday to Friday, 8 am to 4:30 pm, with rotating weekends.
