About the position

The Inpatient Social Worker Care Manager I (SWCM I) plays a crucial role in the collaborative interdisciplinary process of case management within a hospital setting. This position focuses on assessing, planning, facilitating, coordinating care, and evaluating the needs of patients and their families, particularly those facing complex psychosocial challenges. The SWCM I advocates for services and resources to promote quality, cost-effective outcomes, while also providing crisis intervention and mental health assessments as needed.

Responsibilities

  • Facilitate the interdisciplinary process of case management including assessment, planning, and care coordination.
  • Assist patients and families in addressing social, financial, and psychological issues related to health conditions.
  • Identify options and advocate for services to meet comprehensive needs of patients and families.
  • Provide clinical social work and complex discharge planning assistance to patients with psychosocial needs.
  • Assist with eligibility determination for social programs and community assistance.
  • Offer crisis intervention and mental health assessments to patients and families.
  • Coordinate and facilitate the development of multidisciplinary discharge plans for high-risk populations.
  • Participate in interdisciplinary team meetings to address psychosocial issues across the continuum of care.
  • Engage in quality improvement activities and promote a customer-friendly environment.

Requirements

  • Master's degree in Social Work from an accredited school.
  • Licensed Master Social Worker (LMSW) in the State of Texas.
  • Prior experience as a social worker preferred.
  • Above average verbal communication skills and exceptional writing skills.
  • Proficiency in Microsoft Word and PC usage.

Nice-to-haves

  • Related specialty certification preferred.
  • Bilingual in Spanish preferred.
