About the position

The PRN Social Worker position at Lifepoint Rehabilitation involves providing essential social work services within an inpatient rehabilitation unit. The role focuses on ensuring that patients' medical, psychological, and social needs are met while facilitating seamless care transitions and working closely with healthcare teams and insurance carriers. The Social Worker will also assist in case management and discharge planning, contributing to the overall recovery and independence of patients.

Responsibilities

• Work closely with patients, caregivers, and the healthcare team to ensure the patient's medical, psychological, and social needs are met.
• Ensure seamless delivery of care between healthcare systems and interface with insurance carriers to facilitate continued coverage/authorization.
• Assist the Program Director in assessing internal patients for admission to the Rehab Unit.
• Develop case management, social work, and discharge planning services for the Acute Rehabilitation Unit.
• Maintain up-to-date knowledge of changes in community resources and Medicare regulations regarding Acute Rehabilitation Unit.
• Interface effectively between patients, families, and medical professionals.

Requirements

• Master's Degree in Social Work from an accredited college or university.
• Current Social Work license (MSW) or eligibility, if required by state practice laws.
• Experience in the provision of Social Service preferred in acute rehabilitation.
• Clinical experience working with individuals, families, and groups.
• Working knowledge of basic services provided by various health and welfare organizations in the community.
• Current CPR certification, if required by the facility assigned to.
• Strong leadership skills.

Nice-to-haves

• Knowledge of CARF standards and Medicare regulations for inpatient acute rehabilitation.
• Experience in a healthcare setting, such as acute care and/or rehabilitation.
