About the position

The Program Case Manager/Patient Care Coordinator plays a crucial role in a multidisciplinary clinic specializing in chronic pain and prescription drug abuse. This position involves coordinating patient care, interfacing with various stakeholders, and supporting physicians in delivering quality care to patients. The role requires strong communication skills and the ability to manage patient information effectively.

Responsibilities

• Interfacing with referring medical clinics, physicians, insurance adjusters, and patients.
• Supporting physicians in the delivery of care to patients as part of a multidisciplinary team.
• Obtaining authorizations for treatment and medical records.
• Verifying patient information, conducting history taking, and confirming current conditions.
• Coordinating patient care with the scheduler and arranging patient transportation.
• Counseling patients on physician orders and updating patient information in the EMR system.
• Following up on medical appointments, authorizations, and requests for medical records.
• Providing support to coworkers and being cross-trained to back up co-workers.

Requirements

• High School Graduate
• Strong writing skills
• Bilingual in English and Spanish
• 1-2 years of medical office and/or clinical experience, preferably in Occupational Health or Urgent Care.

• Medical Administration and/or Medical Assistant Certificate
• Strong communication skills, both written and verbal
• Team player
• Consistently positive attitude
• 2-5 years of medical office experience in Patient Case Coordination.
